3 1/4" x 4 3/8" Dedication Order Form $25.00 Cash or check payable to: JL Long PTA Please use one of the layouts below.  All writing, typing, photos or design work must  fall inside the black borders.  Return payment and this sheet with completed, camera-ready design to the Yearbook Table in the Cafeteria. Yearbook Table will be open every Friday during all lunches starting September 25, 2009 through February 26, 2010. Due to Friday holidays or early release, Yearbook Table will be open during lunch on the following Thursdays: October 8th October 22nd ** January 28th					Forma Dedicacion de Pedido -- $25.00 cheques a favor de "J.L. Long PTA"   Toda la escritura, mecanografia, fotos, y el trabajo de diseno debe caer dentro del bordo negro.   Porfavor use uno de los disenos de abajo.   Regrese esta hoja completa con, el diseno lista camara a la mesa Libroanual en la cafeteria. La mesa del Libroanual estara abierto todo los Viernes durante el almuerzo comensando Septiembre 25, 2009 hasta Febrero 26, 2010.   Debido a Viernes y los dias festivos, o dias de salida temprano,  la mesa del Libroanual estara abierto durante el almuerzo los dias Jueves sigientes: Octubre 8 Octubre 22 ** Enero 28, 2010.
